Abstract

Background: Hypertension is a chronic disease that requires lifelong treatment to prevent complications such as stroke, heart disease, and kidney failure. However, medication adherence among patients with hypertension remains low due to inadequate knowledge and awareness regarding the importance of continuous treatment. Health education is one of the nursing interventions that can improve patients' knowledge, awareness, and adherence to antihypertensive medication. Purpose: This study aimed to determine the effect of health education on medication adherence among patients with hypertension at Andong Primary Health Center. Methods: This study employed a pre-experimental design using a one-group pretest-posttest approach. A total of 33 respondents were selected through purposive sampling. Data were collected using the Morisky Medication Adherence Scale (MMAS-8). Data were analyzed using the Wilcoxon Signed Rank Test because the data were not normally distributed. Results: Before receiving health education, most respondents had low medication adherence (54.5%), followed by moderate adherence (39.4%) and high adherence (6.1%). After the intervention, high adherence increased to 57.6%, moderate adherence remained at 39.4%, and low adherence decreased to 3.0%. The Wilcoxon Signed Rank Test showed a significant difference between pretest and posttest medication adherence (p-value = 0.001; p < 0.05), indicating that health education significantly improved medication adherence among patients with hypertension. Conclusion: Health education has a significant positive effect on improving medication adherence among patients with hypertension. Integrating structured health education into routine nursing practice in primary healthcare settings is recommended to enhance treatment adherence and reduce hypertension-related complications.
